Improve Loop unrolling (#6480) (#6493)
This patch implements #6480. All loop statements are represented using
AstLoop and AstLoopTest.

This necessitates rework of the loop unroller to handle loops of
arbitrary form. To enable this, I have split the old unroller used for
'generate for' statements and moved it into V3Param, and subsequently
rewrote V3Unroll to handle the new representation. V3Unroll can now
unroll more complex loops, including with loop conditions containing
multiple variable references or inlined functions.

Handling the more generic code also requires some restrictions. If a
loop contains any of the following, it cannot be unrolled:
- A timing control that might suspend the loop
- A non-inlined call to a non-pure function

These constructs can change the values of variables in the loop, so are
generally not safe to unroll if they are present. (We could still unroll
if all the variables needed for unrolling are automatic, however we
don't do that right now.)

These restrictions seem ok in the benchmark suite, where the new
unroller can generally unroll many more loops than before.

Fix toggle coverage aggregation on same line (#5248)
Documentation states that minimum of all reported coverage of all signals in a line should be taken.
Previous logic would break if there were any signals with zero coverage followed by signals with
nonzero coverage - a minimum from those nonzero toggle count would be taken, disregarding zero
coverage of previous signals.

Emit model implementation as loose methods. (#3006)
This patch introduces the concept of 'loose' methods, which semantically
are methods, but are declared as global functions, and are passed an
explicit 'self' pointer. This enables these methods to be declared
outside the class, only when they are needed, therefore removing the
header dependency. The bulk of the emitted model implementation now uses
loose methods.

Add lint_off -match waivers (#2102)
* Add more directives to configuration files

Allow to set the same directives in configuration files that can also
be set by comment attributes (such as /* verilator public */ etc).

* Add support for lint messsage waivers

Add configuration file switch '-match' for lint_off. It takes a string
with wildcards allowed and warnings will be matched against it (if
rule and file also match). If it matches, the warning is waived.

Fixes #1649 and #1514
